Strange trade reversal behavior on Steam API, items not returned to inventory?

Hey, ran into some strange behavior this past week related to trade reversals across two different Steam accounts. Wanted to see if anyone else has observed this. Both of these items were traded on the same day. 29'th of last month as you can see from the images.

Account 1: No reversal status (status 12) was ever detected on the Steam API, no notification, no email, nothing in trade history. My program only logged Item not found in inventory for a Hydra Gloves. I received compensation from the site I was trading on. The original trade later appeared to go through normally today. And that item is still not in my inventory and there's no trace of a reversal anywhere on Steam's side.

Account 2: Got a proper Steam notification, a Steam email, and the reversal shows up in Steam trade history. However, in the trade history it's showing a - sign even though I was the one who sent the trade (should be + if items were returned to me right?). The item (Butterfly Knife Fade) is not in my inventory either. Also received compensation from the site here.

Questions:

• Has anyone seen items disappear from inventory with no reversal status on the API, no notification and no trade history entry, but compensation still triggered on the site?
• Has anyone seen reversed items simply not appearing in inventory after a confirmed reversal?
• Is the Steam trade history API currently reliable for reversal tracking or are there known issues?

Would appreciate any input.
